A Team-Based Approach To Healthcare


A patient-centered medical home (PCMH) is a primary care facility following a team-based approach to healthcare. It is a model of practice in which a team of health professionals, coordinated by a personal clinician, works collaboratively to provide high levels of care, communication, care coordination and integration, and care quality and safety.


Using Existing Healthcare Assets To Benefit Patients

The PCMH practice works to overcome any barriers that might prevent full communication with a patient in order for them to be a partner with the physician in his/her own medical care. Often, this means that visits may be scheduled for longer than the traditional 15-minute appointment in order to allow the time to discuss treatment plans and testing options with the patient.

Preventative services and integrated management of chronic diseases can improve the quality of care and lower costs. InHouse Physicians offers a patient centered medical home model for members with chronic or complex diseases. The patient centered medical home will go through a recognition process via the accreditation body NCQA (National Committee for Quality Assurance). This approach facilitates our on-site clinicians in closing gaps for the highest risk patients leading to improved clinical outcomes and decreased associated claims.
